Resolution includes documented outcomes, corrective actions, and, where applicable, policy changes and training updates. Actions should be timely, proportional, and transparent to the reporter, while preserving confidentiality.
Communication with stakeholders should be appropriate and non-identifying. Provide the minimum necessary information to affected parties and teams, respecting privacy constraints.
Continuous improvement relies on data and feedback. Track metrics, review procedures, update training, and embed learnings into policies and daily practices. Engage leadership to sustain improvements.
