Description
This module covers interview techniques, evidence collection, objectivity, and documentation standards. You will practice steps from intake through findings, ensuring that due process is respected and records are ready for potential audit or legal review.
To determine what happened based on credible, verifiable evidence while upholding due process, neutrality, and privacy. The goal is to identify facts and determine appropriate remedies.
Evidence includes documents, emails, chat transcripts, calendars, receipts, and other records that are authentic and relevant. Interviews conducted and documented must also be considered, with a clear chain of custody maintained.
Plan by defining scope and objectives, identifying interviewees, obtaining consent for recording, choosing appropriate locations and times, and setting a realistic timeline with a standard interview guide.
Use a structured, neutral interview guide, ask open ended questions, allow silence for reflection, avoid leading prompts, and consider multiple interviewers or peer reviews to check for consistency.
Cross check with other sources, document inconsistencies clearly, seek clarifications where possible, and avoid drawing conclusions from a single statement without corroboration.
Limit access to sensitive information, store records securely, de-identify data where appropriate, and comply with data protection laws and organizational policies regarding confidentiality.
Set milestones for intake, interviews, evidence collection, draft findings, and final reporting. Communicate expected timelines to stakeholders and provide updates if delays occur.
Present facts with sources, separate facts from inferences and conclusions, attach evidence mappings, ensure readability and an auditable trail, and seek appropriate reviews before finalizing.
Protect complainants and witnesses with anti retaliation measures, monitor for retaliation, escalate if needed, and document steps taken to preserve safety and trust.
Provide a safe, respectful environment; explain rights and process; avoid coercion; offer accommodations when needed; conduct interviews separately to reduce influence.
Preserve original copies and metadata, avoid altering sources, maintain a clear chain of custody, and verify authenticity and relevance before including in the record.
Prioritize fairness and accuracy while avoiding unnecessary delays. Escalate urgent safety concerns promptly and document the rationale for scheduling decisions.
Provide a concise, fact based report with actionable remedies and timelines. Protect confidentiality and avoid naming individuals publicly unless appropriate, and share outcomes with the right stakeholders.
Bias in interviewing, incomplete evidence collection, poor documentation, confidentiality breaches, scope creep, and failing to follow policies or legal requirements.
Findings inform policy refinements, training needs, and corrective actions. Align remedies with root causes and business risk, and establish follow up and accountability.
State the limits of the evidence clearly, propose monitoring or interim controls, and outline next steps or additional avenues for information gathering.
Ideally an independent or trained investigator; internal HR or Legal can lead with appropriate safeguards. External investigators can help reduce perceived bias; ensure qualifications and neutrality.
Maintain an auditable trail, secure storage, retention schedules, and privacy compliance. Document chain of custody, access logs, and decisions to prepare for internal and external reviews.
This quiz tests your understanding of fair investigation practices focusing on evidence collection, interview techniques, and documentation. It covers chain of custody, bias awareness, confidentiality, and proper reporting to support credible outcomes in harassment and diversity investigations.
